This is the main person you will see for most of your health care. This includes checkups, treatment for colds and flu, health concerns and health screenings. Your PCP can find and treat health problems early. He or she will have your medical records. Your PCP can see your whole health care picture. Your PCP keeps track of all the care you get.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are different types of doctors who are PCPs, like:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">General Practitioners<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Family Physicians<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Pediatricians<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Gynecologists\/Obstetricians<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Internists<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">You must choose a PCP for each insured member in your family. Your family members can have different PCPs.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you are a woman over age 12, you can also choose a gynecologist to be your PCP. If you are pregnant, your PCP could be your obstetrician during your pregnancy. When your pregnancy ends you will go back to your regular doctor, but your gynecologist will still take care of your gynecological needs. You may choose a pediatrician or a family physician for your newborn or one will be chosen for you.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">To choose your PCP, call Triple-S Salud at <\/span><a href=\"tel:18009811352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">. If you do not choose one, then one will be chosen for you.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">A Primary Medical Group is a group of doctors that help arrange your health care services and work with your Insurer to make sure you get the care you need. Your ID card shows the name of your PCP and your Primary Medical Group number.<\/span><\/p>\n<h4>HOW CAN I SEE MY PCP?<\/h4>\n<p><span style=\"font-weight: 400;\">If you need an appointment, call your PCP. It is free to make appointments with them. It is important that you keep your appointments with your PCP. If you cannot make it for any reason, call the PCP\u2019s office right away to let them know.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If your PCP is new for you, you should get to know him\/her. Call to get an appointment as soon as you can. This is even more important if you\u2019ve been getting care or treatment from a different doctor. We want to make sure that you keep getting the care you need. If you feel OK, you should call to get a checkup with your PCP.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Before you go to your first appointment:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Ask your past doctor to give you your medical records. This is free of charge. Bring your medical records to your new PCP at your first visit. They will help your new PCP learn about your health.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Call your PCP to schedule your appointment.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Have your Member card ready, when you call your PCP.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Say you are a Plan Vital member and give them your MPI number.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Write down your appointment date and time. If you\u2019re a new patient, the provider may ask you to come early. Write down the time they ask you to be there.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Make a list of questions you want to ask your doctor. List any health problems you have.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If you need a ride to the appointment and have no other way to get there, call your plan or your local Municipality. They can help you get a ride.<\/span><\/li>\n<\/ol>\n<p><span style=\"font-weight: 400;\">On the day of your appointment:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Bring a list of all your medicines and your questions with you so your doctor will know how to help you.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Be on time for your visit. If you cannot keep your appointment, call your PCP to get a new time.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Take your ID card with you. Your PCP may make a copy of it.<\/span><\/li>\n<\/ol>\n<h4>WHAT IF IT\u2019S AN EMERGENCY AND I NEED CARE AFTER MY PCP\u2019S OFFICE CLOSES?<\/h4>\n<p><span style=\"font-weight: 400;\">Most PCPs have regular office hours. Triple-S Salud Provider Directory will tell you when your doctors\u2019 offices are open. Most Primary Medical Groups also have clinics that have an extended schedule but, you can call Triple-S Salud Medical Counseling Line at any time.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can get emergency health care any time you need it. Always carry your Plan Vital Member Card with you.\u00a0<\/span><b>In case of an<\/b><span style=\"font-weight: 400;\">\u00a0<\/span><b>emergency<\/b><span style=\"font-weight: 400;\">, doctors will know you have Plan Vital. If you call Triple-S Salud Medical Counseling Line before you go to the emergency room, you will not have to pay when you go to the emergency room.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Triple-S Salud Medical Counseling Line number is\u00a0<\/span><a href=\"tel:1-800-981-1352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Emergencies are times when there could be danger or damage to your health if you don\u2019t get medical care right away.<\/span><\/p>\n<table>\n<tbody>\n<tr>\n<td width=\"350\"><strong>Emergencies might be things like: <\/strong><\/td>\n<td width=\"281\"><strong>These are usually not Emergencies:<\/strong><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Shortness of breath, not able to talk<\/span><\/td>\n<td width=\"281\">Sore throat<\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">A bad cut, broken bone, or a burn<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Cold or flu<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Bleeding that cannot be stopped<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Lower back pain<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Strong chest pain that does not go away<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Earache<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Strong stomach pain that doesn\u2019t stop<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Stomachache<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Seizures that cause someone to pass out<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Small, superficial, cuts<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Not able to move your legs or arms<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Bruise<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">A person who will not wake up<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Headache, unless it is very bad and like you\u2019ve never had before<\/span><\/td>\n<\/tr>\n<tr>\n<td width=\"350\"><span style=\"font-weight: 400;\">Drug overdose<\/span><\/td>\n<td width=\"281\"><span style=\"font-weight: 400;\">Arthritis<\/span><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><span style=\"font-weight: 400;\">If you think you have an emergency, go to the nearest hospital Emergency Room (ER). If you can\u2019t get to the ER, call 911.<\/span><\/p>\n<p><b>If you need emergency care, you don\u2019t have to get an authorization from anyone before you get emergency care.<\/b><\/p>\n<p><span style=\"font-weight: 400;\">If you are not sure if it\u2019s an emergency, call your PCP. You can call your plan\u2019s Medical Counseling Line at any time. Your PCP can help you get emergency care if you need it.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can also call Plan Vital call center for advice. Their phone number is on the back of your ID card. You can call 24 hours a day, 7 days a week.<\/span><\/p>\n<h4>CAN I CHANGE MY PCP?<\/h4>\n<p><span style=\"font-weight: 400;\">Yes, you can change your PCP at least once a year. There are many reasons why you may need to change your PCP. For example, you may want to see one whose office is closer to you. To change your PCP, you must call Triple-S Salud to corroborate whether the change may be performed.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can also change to a new Primary Medical Group if the PCP you want to see is in a different Primary Medical Group.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Most of the time, after the first 90 days of signing up with your plan, you can change your Primary Medical Group at any time for some reasons, like if:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your PCP can\u2019t give you the care or treatment you need because of ethical (moral) or religious reasons.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your PCP can\u2019t give you all the services you need at the same time, and not getting services at the same time is risky for your health.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You get bad quality care.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You can\u2019t access the services you need.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Your PCP doesn\u2019t have experience to take care of your health care needs.<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">For orientation and to make the change, call Triple-S Salud at\u00a0<\/span><a href=\"tel:1-800-981-1352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Another reason why your PCP or Primary Medical Group could change is if your PCP or Primary Medical Group stops working with Triple-S Salud. If this happens, Triple-S Salud will send you a letter letting you know your new PCP or Primary Medical Group. If you want to change your PCP or Primary Medical Group, call at\u00a0<\/span><a href=\"tel:1-800-981-1352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h4>WHAT HAPPENS AFTER I ASK FOR THE CHANGE?<\/h4>\n<p><span style=\"font-weight: 400;\">Once you make the change with Triple-S Salud, it will take some time for the change to be effective. If you make the change in the first 5 days of a month, it will be effective in the next month. For example, if you make the change on January 5, it will be effective on February 1. But if you make the change after the first 5 days of the month, it will be effective the month after next. For example, if you make the change on January 6, it will be effective March 1.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You should keep seeing your old PCP until the change is effective. You cannot start seeing your new PCP until the effective date.<\/span><\/p>\n<h4>WHAT ABOUT OTHER DOCTORS OR PROVIDERS I NEED TO SEE?<\/h4>\n<p><span style=\"font-weight: 400;\">Besides your PCP, you may also need to see other doctors and health care providers, like specialists. A specialist is a doctor who gives care for a certain illness or part of the body. One kind of specialist is a cardiologist, who is a heart doctor. Another kind of specialist is an oncologist, who treats cancer. There are many kinds of specialists.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Besides specialists, you may also need to go to other healthcare professionals and healthcare facilities to get care, like laboratories, x-ray facilities, or hospitals. The doctors, other health care professionals and service facilities that work with Triple-S Salud and your Primary Medical Group are called the Preferred Provider Network.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The other doctors, other health care professionals and service facilities that work with Triple-S are called the General Network. When you sign up with Triple-S Salud, your Primary Medical Group and the Service Centers also have a copy of the lists or you can access <\/span><span style=\"font-weight: 400;\">it <a href=\"\/en\/provider-directory\/\">here<\/a><\/span><span style=\"font-weight: 400;\">.<\/span><\/p>\n<p><b>Preferred Provider Network<\/b><\/p>\n<p><span style=\"font-weight: 400;\">The doctors, other health care professionals and services facilities who work with your Primary Medical Group are called the Preferred Provider Network.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">There are benefits to seeing the doctors, other health care professionals and service facilities in the Preferred Provider Network:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">You can visit any of the doctors and service facilities in the Preferred Provider Network for free.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If you visit the doctors, healthcare professionals and service facilities in your Preferred Provider Network, you don\u2019t need to go to your PCP first to get a referral.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">If you get any of the following services within the Preferred Provider Network, you don\u2019t need your PCP to sign off:<\/span>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Prescription medicine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">Laboratory tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"2\"><span style=\"font-weight: 400;\">X-rays<\/span><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">To get more information about your Preferred Provider Network, you can:<\/span><\/p>\n<ol>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Call Triple-S Salud at\u00a0<\/span><a href=\"tel:1-800-981-1352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Call Plan Vital Call Center at\u00a0<\/span><a href=\"tel:1-800-981-2737\"><span style=\"font-weight: 400;\">1-800-981-2737<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:787-474-3389\"><span style=\"font-weight: 400;\">787-474-3389<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Go to Triple-S Salud\u00a0<\/span><a href=\"https:\/\/prd-wordpress-sssvital.azurewebsites.net\/en\/contact-us\/\"><span style=\"font-weight: 400;\">Service Centers<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Call your Primary Medical Group.<\/span><\/li>\n<\/ol>\n<p><b>General Network<\/b><\/p>\n<p><span style=\"font-weight: 400;\">The general network is the health care professionals and services facilities that work with Triple-S Salud and that support the Primary Medical Groups. If the doctor or provider you need to see isn\u2019t in your Preferred Provider Network, they might be in your Insurer\u2019s General Network. You can see any doctor or provider in your plan&#8217;s General Network as long as you go to your PCP first to get a referral. If you need a referral, your PCP must give you one during your visit or within 24 hours after you ask for one.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Your PCP will coordinate your visits to doctors or providers in the General Network.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You might need to pay money for these visits, click\u00a0<\/span><a href=\"\/en\/copayments\/\"><span style=\"font-weight: 400;\">here<\/span><\/a><span style=\"font-weight: 400;\">\u00a0for more information about payments.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you get any of the following by a provider in the General Network, your PCP will have to sign off:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Prescription medicine<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Laboratory tests<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">X-rays<\/span><\/li>\n<\/ul>\n<p><b>Out-of-Network<\/b><\/p>\n<p><span style=\"font-weight: 400;\">A doctor or other provider who does not work with Triple-S Salud is called an Out-of-Network provider. If you need to see a doctor or other provider who is out-of-network, your PCP must get an OK from your plan first.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you need services from an out-of-network community health clinic, you will first need a referral from your PCP. You can get care at an out-of-network community health clinic for free.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you feel that Triple-S Salud or your doctors are not following these rules, you can call us at\u00a0<\/span><a href=\"tel:1-800-981-1352\"><span style=\"font-weight: 400;\">1-800-981-1352<\/span><\/a><span style=\"font-weight: 400;\">; TTY \/ TDD users should call\u00a0<\/span><a href=\"tel:18552954040\"><span style=\"font-weight: 400;\">1-855-295-4040<\/span><\/a><span style=\"font-weight: 400;\">\u00a0and tell us that you need to make a complaint. You can also call the Patient Advocate Office at\u00a0<\/span><a href=\"tel:1-800-981-0031\"><span style=\"font-weight: 400;\">1-800-981-0031<\/span><\/a><span style=\"font-weight: 400;\">\u00a0or ASES at\u00a0<\/span><a href=\"tel:1-800-981-2737\"><span style=\"font-weight: 400;\">1-800-981-2737<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h4>HELP WITH GETTING TO YOUR HEALTH CARE VISITS<\/h4>\n<p><span style=\"font-weight: 400;\">If you don\u2019t have a way to get to your health care visits, Triple-S Salud and your Municipality can help with transportation. Each Municipality has some ways to help you get to your visits.
