Some examples are:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A person lies about facts to get or keep Plan Vital coverage<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A doctor bills you or makes you pay cash for covered services<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A person uses someone else\u2019s Member card<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A doctor bills for services that you did not get<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">A person sells or gives medications to someone else<\/span><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400;\">If you find out about fraud or abuse, you must tell us about it. You can call Triple-S Salud, the Patient\u2019s Advocate Office or ASES. You do not need to tell us your name and we will keep your information private. You will not lose your Plan Vital coverage if you report fraud or abuse.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">If you want more information, you can visit the ASES website at\u00a0<\/span><a href=\"http:\/\/www.planvitalpr.com\"><span style=\"font-weight: 400;\">www.planvitalpr.com<\/span><\/a> <span style=\"font-weight: 400;\">On the website there is a formulary that you can use to make your report.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">You can also help prevent fraud and abuse. Here are some things you can do:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Don\u2019t give your Plan Vital Member Card to anyone else.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Learn about your Plan Vital benefits.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Keep records of your doctor\u2019s visits, laboratory tests and drugs. Make sure you don\u2019t get repeat services.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Make sure your information is right on a form before you sign it.<\/span><\/li>\n<li style=\"font-weight: 400;\" aria-level=\"1\"><span style=\"font-weight: 400;\">Request and review the quarterly summary of the services you receive.
